Our Comprehensive Package

Whether returning home from the hospital, a skilled nursing facility, or a doctor’s visit, we help our patients achieve independence in their community and safely transfer home with confidence. We help make hospital stays shorter and less frequent by providing healthcare in the home. Our interdisciplinary team partners with patients, their MD, family, and/or caregivers, to achieve goals and quality outcomes (based on the patient’s diagnosis) that enable patients to more easily perform activities of daily living, manage their condition, and move forward equipped with the knowledge and resources they need in their home.

Who is Eligible

Desert Valley Home Health is Medicare certified and works directly with multiple insurance companies. Most insurances, including Medicare, provide these services at no-cost to the patient or family. Eligibility for Desert Valley Home Health Care services is determined by a patient’s physician.The charges are based on actual cost of each service. Those fees may be paid directly by the patient or family. Through many private insurance plans, or through  Medicare or Medical.

Skilled Nurses

 

Our registered and licensed practical nurses provide comprehensive medical care in the home, including:

  • Medication management and education

  • Wound care and dressing changes

  • Chronic disease management

  • Post-surgical care

  • Vital signs monitoring

  • Patient and caregiver education

Physical Therapy

Our physical therapists help patients improve strength, mobility, and balance to safely return to daily activities.

  • Rehabilitation after surgery or injury

  • Fall prevention

  • Pain management

  • Gait and mobility training

Occupational Therapy

Occupational therapy focuses on helping patients regain independence in everyday activities.

  • Activities of daily living (ADL) training

  • Home safety assessments

  • Adaptive equipment recommendations

  • Upper extremity strengthening

Speech Therapy

Speech-language pathologists assist patients with communication, cognition, and swallowing challenges.

  • Speech and language therapy

  • Cognitive rehabilitation

  • Swallowing evaluations and therapy

Home Health Aide Services

Our compassionate home health aides provide assistance with personal care under professional supervision.

  • Bathing and grooming

  • Dressing assistance

  • Light mobility support

  • Personal hygiene

Wound Care

Chronic wounds, such as diabetic ulcers, venous leg ulcers, and pressure injuries, require expert attention. Our team uses advanced therapies and a holistic approach to promote healing and prevent recurrence.

  • Negative Pressure Wound Therapy (NPWT)
  • Hyperbaric Oxygen Therapy (HBOT) (where available)
  • Enzymatic, surgical, and autolytic debridement
  • Advanced dressings and biologic skin substitutes
